As a mobile Exercise Physiology service across Sydney, Bloom provides personalised, evidence-based exercise programs delivered in the comfort of your home, community, gym, school, or aged care facility, or via telehealth. Bloom supports individuals and families across Sydney, specialising in disABILITIES, Oncology, and Aged Care for the management of chronic disease.

We bring all required exercise equipment and can provide sessions in a variety of settings, including:

Supported Independent Living (SIL)

Telehealth (where appropriate)

No! You can book directly without a referral.
Depending on your funding pathway, a referral may be required. For example, if you are accessing Exercise Physiology services through a Medicare Chronic Disease Management (CDM) Plan, a referral from your General Practitioner (GP) is required. Referrals may also be required for some private health insurers

Every exercise program is individually designed based on your health history, current abilities, goals, lifestyle, and any medical considerations.
Your journey begins with a comprehensive initial assessment, where we take the time to understand your medical history, current function, movement capacity, lifestyle, and personal goals. This enables us to safely and effectively prescribe an evidence based exercise program tailored specifically to your needs.
There is no one-size-fits-all approach. Programs are regularly reviewed, progressed, and modified to ensure they remain safe, effective, and aligned with your goals as your needs and abilities evolve.

Your initial assessment allows us to gain a comprehensive understanding of your health, medical history, functional capacity, goals, and any barriers to exercise.
Following the assessment, we'll develop an individualised exercise plan designed specifically for your needs and discuss the best approach moving forward.

Yes!
As part of professional practice, Accredited Exercise Physiologists (AEPs) are required by Exercise & Sports Science Australia (ESSA) to maintain accurate and comprehensive clinical documentation.
Clinical documentation may include:
Progress notes
Individualised exercise programs
Outcome measures
Goal reviews
Progress reports
Functional capacity reports
Communication with your healthcare team (where appropriate and with consent)
With your consent, relevant clinical documentation may also be shared with your family members, carers, support workers, case managers, or other allied health professionals involved in your care to support collaborative, person centred care.
All documentation is tailored to your individual clinical needs, funding requirements, and professional reporting standards.

BLOOM brings all necessary exercise equipment to each session. Where appropriate, exercises can also be designed using equipment you already have at home or with minimal equipment.
